According to the Texas Department of Public Safety, a Toyota Highlander traveling south on Highway 155 crossed into the northbound side of the roadway near FM 344 and collided head-on with a Cadillac SUV. Investigators had not determined why the Highlander entered opposing traffic.

The Highlander driver died at the scene. Three adults traveling in the Cadillac were also pronounced dead. A 4-year-old child riding in the Cadillac was initially transported for emergency treatment and later died from injuries sustained in the crash, bringing the total number of fatalities to five.

DPS also reported that none of the occupants was wearing a seat belt and that the child was not secured in a child safety seat. That evidence may be relevant to the severity of the injuries, but it does not answer the separate question of why the Highlander crossed into oncoming traffic and caused the head-on collision sequence described by investigators.

Why Did the Highlander Cross the Center of Highway 155?

That is likely the most important unanswered question in the crash investigation.

Investigators may need to determine whether the Highlander entered the northbound lane because of driver distraction, fatigue, a medical emergency, mechanical failure, an attempted evasive maneuver, roadway conditions, or some other circumstance. None of those possibilities has been established by the available reporting.

Potentially important evidence could include vehicle event data, cell-phone information when legally relevant, witness statements, roadway markings, tire evidence, crash-scene measurements, surveillance or dashcam recordings, and the mechanical condition of the Highlander.

DPS reported that none of the five people involved—including the toddler—was properly restrained at the time of the collision.

That evidence is significant, but it should not be confused with evidence explaining why the crash occurred.

A Dallas woman charged in connection with the 2023 crash that killed Southern Methodist University student Honor Wallace had a blood-alcohol concentration of 0.294% in testing performed after the collision, according to testimony from a toxicology expert at her ongoing criminal trial. Lynlee Pollis, 27, is charged with intoxication manslaughter and intoxication assault in connection with the September 24, 2023 crash. Toxicologist Testifies BAC Was 0.294% Toxicology chemist Brittany Welch testified Thursday that testing performed hours after the crash showed Pollis had a 0.294% blood-alcohol concentration, nearly four times the 0.08% level Texas law defines as intoxicated for purposes of driving offenses. Welch testified that a person would likely have consumed approximately six to 11 standard-sized alcoholic drinks, or three to five larger drinks or doubles, to reach that level. The defense is disputing the intoxication evidence. NBC 5 reported that defense attorneys have challenged how the blood evidence was collected and handled and have argued that Pollis did not appear intoxicated despite acknowledging that she was speeding and ran the red light. The charges remain allegations, and Pollis is entitled to the presumption of innocence unless and until she is convicted. What Happened in the Dallas Crash That Killed Honor Wallace? The collision occurred at approximately 4:20 p.m. on September 24, 2023. Wallace was traveling east on Monticello Avenue and entered the intersection with the North Central Expressway service road. Prosecutors allege Pollis was traveling north on the service road in a Lexus SUV, entered the intersection after the traffic signal had already turned red, and struck Wallace’s Volkswagen Jetta. Evidence presented at trial reportedly indicated that Pollis was traveling well above the posted speed limit before impact and that Wallace’s vehicle was sent briefly airborne by the force of the collision. Wallace died from her injuries. Her passenger survived but suffered serious injuries. Fatal Accident Under I-35 Bridge in Dallas

One person was killed in a single-vehicle crash on I-35E near Camp Wisdom Road in southern Dallas early Monday morning, September 28, 2026. Dallas Police said officers and Dallas Fire-Rescue responded just before 2:30 a.m. to the area of South R.L. Thornton Freeway and East Camp Wisdom Road after a passerby called 911. Dallas Fire-Rescue located one person at the scene and pronounced the victim dead. Police have not publicly identified the person who died, and investigators have not yet explained what caused the vehicle to crash. Fatal Single-Vehicle Crash Near Camp Wisdom Road The collision occurred along South R.L. Thornton Freeway, which carries I-35E through southern Dallas, near East Camp Wisdom Road. Police have confirmed that only one vehicle was involved. A 13-year-old Sanger ISD student died after a school bus left the roadway, struck a culvert, and rolled over in Denton County on Thursday afternoon, September 24, 2026. The single-vehicle crash happened at approximately 5:00 p.m. near Huling Road and Lois Road East. According to the Texas Department of Public Safety, the bus was traveling northbound on Huling Road when it went off the roadway, struck a culvert, and overturned.
