A 28-year-old Fate man was killed and another driver was seriously injured in a head-on collision on Farm-to-Market Road 1138 in Collin County Thursday evening, October 8, 2026.

According to the Texas Department of Public Safety, the crash happened at approximately 7:05 p.m. near Crowell Lane.

DPS identified the deceased driver as Christian Samuel Lamoore Dean, 28, of Fate. Investigators said Dean was driving a 2025 Hyundai Elantra southbound on FM 1138 when, for reasons still under investigation, the Hyundai crossed into the northbound lane and collided head-on with a 2016 Toyota Tacoma.

Dean was pronounced dead at the scene.

The Tacoma driver, identified as 23-year-old Adrian Mateos Aguilar of Dallas, was transported to a hospital with suspected serious injuries.

What Happened in the Fatal FM 1138 Crash?

DPS's preliminary investigation indicates that the Hyundai crossed onto the wrong side of FM 1138 before striking the northbound Toyota Tacoma.

Authorities have not yet said why the Hyundai crossed the center of the roadway. The investigation remains ongoing, and a preliminary crash account may change as troopers review additional evidence.

No conclusion should be drawn about the underlying cause of the collision until that investigation is complete.

Evidence That May Matter After a Head-On Collision

A serious head-on crash often requires investigators to determine not only which vehicle crossed the centerline, but also why it happened.

Potentially important evidence may include:

  • Electronic vehicle data

  • Dash-camera or nearby surveillance footage

  • Tire marks and roadway debris

  • Vehicle damage and impact patterns

  • Cellphone records when relevant

  • Witness statements

  • Roadway markings and lighting

  • Mechanical inspection of the vehicles

This evidence may help determine whether distraction, speed, a mechanical problem, evasive action, a medical event, or another circumstance contributed to a vehicle entering the opposing lane.

Head-On Crashes Can Cause Catastrophic Injuries

Head-on collisions can produce severe forces because vehicles traveling in opposite directions may meet with little opportunity to avoid impact.

Survivors may suffer traumatic brain injuries, spinal injuries, fractures, internal injuries, or other conditions requiring emergency and long-term medical care.

In this crash, DPS reported that the Toyota Tacoma driver suffered suspected serious injuries and was transported for treatment. Hoodline

Wrongful Death and Serious-Injury Claims After a Collin County Crash

A fatal or catastrophic collision may lead to a civil investigation separate from the DPS investigation.

Depending on the evidence, that investigation may examine electronic vehicle data, available video, witness testimony, insurance coverage, roadway evidence, and any other circumstances that help explain why the collision occurred.
